Local fibrinolysis in patients at high risk of pre and post operative hemorrhage following prostatectomy, epistaxis, conization of cervix, menorrhagia; hereditary angioneurotic edema and management of dental extraction in hemophiliacs.
Doses & Administrations:
Children and adults: IV ;10mg/kg immediately before surgery, then 25mg/kg/dose orally 3-4 times per day for 2-8 days. Alternatively; Oral: 25mg/kg 3-4 times/day begining 1 day prior to surgery IV : 10mg/kg 3-4 times/day in patients unable to take oral. MAXIMUM ORAL DOSE: 6gm per day (Adults) - MOUTH WASH: Use solution of 1-2 ampules (undiluted) up to 4 times a day. Swish and spit.....Do not swallow.
Contra Indications:
History of thrombo-embolic disorders, active intravascular clotting process.
Precautions:
Renal impairment, hepatic dysfunction.

PARENTERAL: Serum creatinine (mcmol/L) Intravenous dose - 120 to 250 10 milligrams/kilogram (1.36 to 2.83 mg/dL) twice daily 250 to 500 10 milligrams/kilogram (2.83 to 5.66 mg/dL) once daily over 500 10 milligrams/kilogram (over 5.66 mg/dL) every 48 hours ORAL: Serum creatinine (mcmol/L) Tablets -120 to 250 15 milligrams/kilogram (1.36 to 2.83 mg/dL) twice daily 250 to 500 15 milligrams/kilogram (2.83 to 5.66 mg/dL) once daily over 500 15 milligrams/kilogram (over 5.66 mg/dL) every 48 hours or 7.5 milligrams/kilogram every 24 hours

Nursing Considerations:
Tranexamic acid injectable solution should be stored at room temperature (15 to 30 degrees C). A continuous intravenous infusion of tranexamic acid (2000 milligrams in 500 milliliters of Dextrose 5% in water, given every 8 hours for 6 days) was reported effective in reducing hemorrhagic episodes. INTERMITTENT INFUSION: DILUTE IN 50 ML SOL. AND INFUSE OVER 30 MINUTES. MAX. RATE = 100 mg/minute Compatible solution is D5W.
